The following forms can be downloaded and printed.
Complete Membership Application and provide:
- Opening deposit (Minimum $5.00)
- Copy of your driver's license(s)
Complete Checking Application and provide:
- Opening deposit (Minimum $50.00)
- Copy of a deposit slip or "voided" check from your existing account
Complete application and provide income verification
- Employee - Current Paycheck Stub and 2014 W-2
- Self employed - 2013 and 2014 personal (and corporate) tax returns
- Retired - Copy of statement showing direct deposit or copy of award letter
On-Line Loan Application or Print/Paper Consumer Loan Application or Print/Paper Residential Loan Application
Additional papers required for processing:
Vehicle Purchase
- Purchase agreement from dealer
- Or copy of the title/registration card from owner
Vehicle Re-finance
- Payment book showing account # and phone #
- Title/registration card of vehicle
- Insurance information
Home Equity
- Statement/balance of existing mortgages
- Homeowners insurance policy
